UWNC is a part of the UW Medicine Health System; we are affiliated with UW Medical Center, Harborview Medical Center, Northwest Hospital & Medical Center, Valley Medical Center, UW Physicians, the UW School of Medicine and Airlift Northwest. UW Neighborhood Clinics is a not for profit, community-based, physician-led primary care serving patients in 12 clinic locations throughout the greater Seattle Area, Olympia and Smokey Point.

The Registered Dietitian Nutritionist provides clinical care to patient which includes nutrition assessment, education, and therapeutic nutritional management services to patients. Is responsible for individual patient counseling in nutrition and diet therapy as well as group education classes focused on various health conditions.  Serves as a nutrition resource person for staff and patients.  Is a member of the care team contributing expertise in medical nutrition therapy.  Ensures that the highest quality of clinical practice, judgment and integrity are upheld and implemented.

The Diabetes and Nutrition Center Quality Coordinator ensures implementation of the Standards and oversees the DSMES services. The quality coordinator is responsible for all components of DSMES, including evidence-based practice, service design, evaluation, and continuous quality improvement. The quality coordinator assumes a lead role in all components of Nutrition services, including program development, evidence-based practice, service design, and continuous quality improvement.

SALARY: $34.18 - $46.14 DOE

Required:   Bachelors degree in nutrition/dietetics, or related health field
Desired:       Masters in Nutrition, Education or related health field                                  

Required: Experience in program development and management of individuals with chronic disease.    Desired:    Expertise in diabetes education. Proficiency in diabetes medication adjustment and recommendation. Expertise in Integrative Nutrition.

Required: Appropriate/current license in professional field.  Desired:   Registered Dietitian Nutritionist, Certification in Diabetes Education from the National Certification Board for Diabetes Educators (NCBDE) or in process/willingness to becoming certified. Certification in Integrative Nutrition.

We have an outstanding opportunity for a Diabetes Care Clinical Nurse Specialist to join our team at University of Washington Medical Center.   The Diabetes Clinical Nurse Specialist: 
• Provides consultation to clinical teams, including physicians, advance practice providers, pharmacy, electronic medical record analysts, patients and families, and nursing staff related to diabetes and/or glycemic control. 
• Collaborates with other providers and clinical resources to ensure coordination and continuity of diabetes care across settings and over time. 
• Facilitates collaboration with the Inpatient Glycemic Team and Outpatient Diabetes Care Clinic to ensure continuity of care. 
• Provides patient and/or family with health education, diabetes self-management education (DSME), counseling and referrals to other health professionals and community resources as appropriate. 
• Participates as a key member of the interdisciplinary team through the development of collaborative and innovative glycemic care practices and evaluation of patient care outcomes. 
• Evaluates quality outcomes through participation in the development of glycemic control quality indicators. Member of the UW Medicine Diabetes Strategic Pathways Committee
• Oversees Subcutaneous Insulin Pump, Continuous Glucose Monitor and other diabetes technology usage at UWMC. 
• Ensures nursing education is current regarding diabetes/glycemic control best practices.
• Participates on Surgical Blood Glucose Management Committee to ensure safe and therapeutic glycemic control for patients through the peri-op period. 
• Provides consultation and support to Med-Specialty Clinics for assistance with glycemic management of patients after solid organ transplant. 

Position is 80%, days.  Requirements: 
• Graduate degree from an accredited nursing education program with a CNS or NP focus 
• Training and experience in diabetes management across the continuum 
• Proven excellence working in an interdisciplinary collaborative team environment 
• Effective interpersonal and leadership skills 
• Experience providing diabetes self-management education (DSME) 
• Experience providing interdisciplinary staff education 
• Active and unencumbered Washington State RN license 
• Active and unencumbered Washington state Advance Practice Nurse Practitioner license 
Preferred skills: 
• Certified Diabetes Educator (CDE) or board certified Advanced Diabetes Management certifications 
• Experience with diabetes technologies including subcutaneous insulin infusion pumps, continuous glucose monitors, closed loop systems, and artificial pancreas developments.
