Bachelor’s degree in clinical nutrition, food and nutrition, or related area required. Prefer Certified Diabetic Educator (CDE) and two or more years of clinical dietitian experience to include working with both hospital inpatient and outpatient populations and individuals with diabetes. Master’s degree also preferred.

The Clinical Diabetes Educator participates in the assessment, planning, implementation, and evaluation of the diabetes education program at the Swedish Diabetes Education Center.

In this position you will:

  • Educate patients about diabetes, self-management in the individual or group settings
  • Assess patients self-care needs and his/her ability to manage diabetes
  • Plan and conduct continuing education programs in diabetes for allied professionals and the general public
  • Assess the needs of the patient and family, formulating educational plans, implementing the plan and revises as needed
  • Communicate information and coordinate patient care with appropriate members of the health care team

Required qualifications for this position include:

  • Current state Registered Nursing LicensureMedical
  • Certified Diabetes Educator or ability to obtain certification within 1 year
  • Previous nursing experience

Preferred qualifications for this position include:

  • Bachelor of Science in Nursing
  • Previous nursing experience in a community health or ambulatory care setting
  • Experience with teaching and public speaking

UW Neighborhood Clinics (UWNC) is looking for a Manager of Nursing and Care Management to participate in the development and advancement of the clinical support infrastructure in primary care areas to include care coordination for high risk individuals with chronic illness..

The Manager actively participates with the leadership team for Nursing and Care Management to develop: 1) strategies to ensure care is coordinated between multiple providers as well as settings; and 2) processes to ensure patients receive necessary acute care appointments to minimize preventable emergency room visits and hospitalizations. 

The ideal candidate will have a Bachelors of Nursing, Masters of Nursing degrees, or Masters Degree in a health related field. Ability to deliver care management services or supervising nurses conducting care management services.  Experience with an EMR, particularly Epic, is highly desirable.

UW Neighborhood Clinics (UWNC) is seeking a skilled nurse for the position of RN Care Manager, an integral member of the care team to provide care management and coordination services throughout the continuum of care.

The RN Care Manager supports complex patients by facilitating the patient's implementation of the medical treatment plan through patient-centered education of disease processes and wellness opportunities, and through self-management skill development.  The nurse assists patient access to appropriate health care services and community resource and fosters communications for the benefit of the patient.  The nurse may teach classes or conduct group visits with other team members such as the social work or dietitian.  The RN will also provide ambulatory nursing support as a member of the care team which may include procedures, walk-in triage or provide consultation to the care team staff regarding clinical issues. 

The ideal candidate will have a BSN, care management experience and at least 3 years of nursing. Demonstrated experience with Windows and Microsoft Office applications is required. Experience with an EMR, particularly Epic, is highly desirable.
